Transition - where can I get support?

My child is just going into the Transition stage and I'm not at all confident firstly about the information I will get or whether the right provision will be suggested and then put into place. I don’t know where to go for support.

I hope you'll find it useful. As you'll see it contains lots of great ideas from discussions on this forum about transition to adulthood and transition between schools.

At age 14 at their review ask for a person centred plan – to be reviewed regularly. See www.dimensions-uk.org
